What You need for your appointment

If you have a condition recognized by the MMCC as being treatable with marijuana and you would like to use marijuana to treat your condition, then follow these 5 simple steps:

1.) Call 443-484-7573 to schedule your appointment

Due to the overwhelming interest in medical marijuana our lines frequently go to voice mail. Do not worry, we check our messages throughout the day and will return your call as soon as we can.

2.) Bring all your medical records to your appointment.

Medical documentation is required by the State of Maryland prior to issuing a written certification for the use of medical marijuana. Medical documentation may include the last 12 months of progress notes from your primary care physician or specialist.

In order for our certifying physicians to issue you a written certification for the use of medical marijuana, you must have a qualifying condition. Your medical records must clearly document that you have the condition that you are seeking help with by the use of medical marijuana. Our certifying physicians will review your records prior to your visit to ensure that your condition meets the MMCC's requirements for a written certification to use medical marijuana.

3.) Visit mmcc.maryland.gov and register as a patient prior to your visit

A written certification for the use of medical marijuana can not be issued to you unless you have registered with the Maryland Medical Cannabis Commission.

4.) CONFIRM THAT OUR OFFICE HAS RECEIVED YOUR MEDICAL RECORDS.

5.) Arrive for your appointment 15 minutes early to complete some paper work and have a valid photo ID-drivers license, passport, state ID.


  • Valid Photo ID required-drivers license, passport, State ID
  • Register with the MMCC PRIOR TO YOUR VISIT
  • Confirm that we have received your medical records

Conditions That can be treated with Marijuana/Cannabis

There are many conditions approved for treatment with medical marijuana. ANY condition that is severe ,failed traditional treatments ,and is reasonably expected to be relieved by the use of marijuana may be treated with marijuana in the state of Maryland. The following conditions may be treated with marijuana:
